Medical malpractice and medical errors are one of the top causes of death and injury in the United States. Occurrences of medication errors resulting in adverse drug reactions and events in hospitalized children are comparable to rates for hospitalized adults. However, the rate for "potential" adverse drug events was three times higher in hospitalized children. The shocker is that the rate for "potential" adverse drug events was substantially higher for babies in neonatal intensive care units. Studies of medical errors outside the hospital are just getting underway across the U.S.
