Motorcycle Wreck Claims Life of Lebanon, TN Woman

A motorcycle wreck in White County, TN takes the life of a Lebanon native and injures another. The couple were headed north on Highway 84 going around a curve when the driver of the motorcycle struck the guardrail, throwing the passenger off, leading to the death of the passenger and severely injuring the driver. Both riders were wearing helmets.

The Hoeganaes plant in Gallatin faces their third fire in 2011. The plant handles combustable metal dust that is extremely flammable. The latest fire that occurred was not like the other two but still left five workers injured. A four person investigations from the U.S. Chemical Safety Board are investigating the fires. Hoeganaes is facing fines up to $42,000 due to the fires.

Motorcycle Wreck Leaves Tennessee Man Critically Injured

A young boy injured in a car wreck was awarded $4.4 million in damages. Xavier Massey was a passenger in an SUV when it stuck three light poles during a storm. Xavier sustained no injuries except for the light pole that sent electriction through his body. The Massey family sued Duke Power Company for negligence because of the faulty light pole. The family received a verdict by jury of $4.4 million in damages.

Helicopter Crash on Indiana University of Pennsylvania's Campus

FDA Recalls Teva Acid Reflux Mediciation

The Food and Drug Administration has recalled the medication called lansoprazole that is manufactured by Teva. The medicine is a generic form of Takeda Pharmaceutical Co's Prevacid. The reports state that any patients using the orally disintegrating tablet (ODT) may run into clogging problems because the drug does not fully disintergrate. No injuries have been reported to the FDA.

The cause of the cracks is still under investigation.

Fatal Truck Crash on I-40

A crash involving two trucks on I-40 has left one man dead.
The crash which took place in Cumberland County, Tn, is thought to be caused by the traffic backup related to a previous wreck farther down the interstate.
The victim, who was driving a 2007 Freightliner van smashed into the back of a 2006 International truck that had stopped or slowed down for the backed up traffic.
While both men were wearing seatbelts, the collision was hard enough to injure the man in the second truck even though he had two trailers of refrigerated food between him and the victim's truck.

Dangerous Munchkin Bathtub Toys Recalled

Macon County Man Dies in Vehicle Accident

A Macon County man was killed in a vehicle accident in Trousdale County. The man was a passenger in a 1999 Plymouth Neon driven by another Macon County man. While traveling north bound on Highway 10 South in Trousdale County, the car veered off of the right side of the road and struck a tree at around 2:30 a.m. The passenger was transported to Trousdale County Medical Center, in Hartsville, where he was pronounced dead on arrival. The driver was eventually transported to Vanderbilt for treatment.

Prominent California Horse Breeder Dies in Plane Crash

Prominent California horse breeder Frank Vessels was killed in a plane crash in Oregon.
Mr. Vessels was piloting a twin engine Aero Commander Model 500-B from Redding California to Montana when the aircraft broke up in mid air and was destroyed after crashing 80 miles South of Burns, Oregon.
One passenger flying with Mr. Vessels was also killed in the crash which is being investigated by the FAA and NTSB.

Mt. Juliet Teen Killed in Single Vehicle Accident

A Mt. Juliet teen was killed when she was ejected from the vehicle, in which she was a passenger, during a single vehicle accident on Greenhill Road in Wilson County.
The vehicle was driven by another female, age 17, of Old Hickory.
Officers on the scene reported that the 2003 Mercury Mountaineer overcorrected after veering across the center line. This sent the car into a yaw that caused it to leave the road and flip into the yard at 1120 N. Greenhill Road.
The passenger, who was not wearing her seatbelt, was ejected from the vehicle and later died at Vanderbilt Children's Hospital. The Driver suffered undisclosed injuries from the accident in which she was wearing a seatbelt.
No charges had been filed at the time of this report.

Two Teens Injured in Head-On with Semi

Two teens were injured when they struck a semi head-on after crossing the center line on Highway 84 in Overton County.
The two teen brothers were driving a Ford Ranger and suffered some broken bones causing one to be sent to Livingston Regional Medical Center, while the other was airflighted to UT Knoxville.
While the semi was damaged, the driver was not injured in the crash.
It was not clear at the time what caused the teen to veer into the oncoming lane of traffic.

Pilot Dies in Georgia Small Plane Crash

A man was killed when his Piper PA 32 went down outside Washington, Georgia.
The pilot, the only occupant of the plane, was en route from Madison County Executive Airport in Huntsville, Alabama to Mount Pleasant Regional Airport in Mount Pleasant, South Carolina.
The crash occured shortly after the pilot reported engine trouble over the radio.
The GBI has investigated the scene and are now awaiting officials from the FAA and NTSB.

Tractor Trailer Hits Van on I-65

A van full of people was hit by a semi while traveling north on I-65.
At least one person was flown by helicopter to Vanderbilt Medical Center from the accident scene.
The accident took place north of Nashville in Maury County near mile marker 49.

Two Die When Plane Hits House in Pennsylvania

A Pennsylvania man narrowly escaped death when a twin engine Beechcraft Baron BE58 crashed into his living room.
The plane, which took off from Arnold Palmer Regional Airport 50 miles away, was carrying the pilot and one passenger who died in the crash.
The pilot was said to have just purchased the plane and was flying with an instructor for the first flight when the crash occured.
The plane was seen spinning in on its own axis before impacting the Saltsburg area home.
The FAA and NTSB are investigating the crash.

Police Captain Struck on I-24

A Metro Police Captain was injured when he was struck by another vehicle while sitting in his cruiser at a road construction site.
The woman driving the other vehicle never attempted to sweve or apply her brakes before striking the captain who was sitting in his vehicle with the emergency lights activated.
The woman was uncooperative and talking on her cell phone while emergency workers were freeing her from her car which she was pinned in for at least 30 minutes.
Her failure to avoid the cruiser with it's blue lights activated coupled with her actions after the accident led officers on the scene to suspect she had been driving under the influence.
Both parties were taken to Vanderbilt Medical Center where the woman was listed in stable condition with serious injuries, and the captain was being examined after complaining of neck and back pain.

Three Killed in Tulsa, OK Plane Crash

A Cesna twin engine plane crashed one mile north of Tulsa International Airport killing all three men on board.
The plane was en route from Pontiac, Michigan to Jones Riverside Airport in Jenks when it radioed Tulsa International and asked permission to land there. The pilot stated that the plane was low on fuel.
Two of the men were killed in the crash while one died later at nearby St. John Medical Center.
It is not yet clear if the low fuel was the cause of the crash.

A Pilot and His Three Grandchildren Killed in Michigan Plane Crash

A pilot and three of his four grandchildren were killed when their Beechcraft Model 58 crashed onto I-75 in northern Michigan shortly after takeoff.
His three granddaughters and one grandson had been visiting from Isreal at the time of the crash.
The only survivor of the crash, the 13 year old grandson, is in critical condition at a local hospital with severe burns.
The plane went down during a short day trip to Mackinac Island.
The cause of the crash is still under investigation.

Fatal Motorcycle Accident Kills Hartsville Man at Macon County Intersection

A motorcycle accident on the Fourth of July took the life of a Hartsville, Tn man in Macon County.
The man was killed when an SUV carrying a family of four made a left turn into the path of his motorcycle at the intersection of Highway 52 West and Oakdale Road in Macon County, TN.
The man died enroute to Macon General Hospital. The family in the SUV were shaken but uninjured in the crash.

Woman Dies in Head-on Collision on Myatt Drive

Myatt Drive, in Nashville, TN, was the scene of a fatal head-on collision that cost one woman her life.
Another motorist and his passenger witnessed the accident after narrowly avoiding the vehicle comming straight at them. The man in the vehicle behind them was not as fortunate and was hit head-on by the woman's car.
The woman was pronounced dead at the scene while the man in the other vehicle was taken to the hospital with injuries to his lower body.

Church Van Crashes in Morristown, TN

Fourteen people were injured when their church van crashed in Morristown, TN.
The van, which was carrying mostly teens from New Vision Ministries in White Pine, Tn, ran a stop sign at a railroad crossing before crashing into an embankment.
Twelve of the injured passengers were taken to Morristown-Hamblen Healthcare System, while two others were airlifted to U.T. Medical Center in Knoxville. Most of the injured were treated and released while three remained in the hospital.
The driver of the van, Cecil Garrett, was cited for running a stop sign, driving on an expired registration and not wearing a seatbelt.

Elderly Woman Struck by MTA Bus in Nashville, TN

A 72 year old woman is still in critical condition at Vanderbilt Medical Center after being struck by a Nashville MTA bus. The woman was crossing Charlotte ave in the crosswalk as the bus was turning onto Charlotte from 4th ave. It was unclear at the time of the accident what color the lights were and if the signals were indicating that it was safe to cross the street.
Neighbors said that the woman regularly walked to the store from her James Robertson apartment.

Plane crash kills four in Arkansas

Two couples, Mike and Terri Hunt and Carl and Carolyn Davis, were killed when their 1977 Piper single engine plane went down in Howard County, Arkansas on Sunday.
The couples took off from DeQeen Arkansas en route to Gaston Arkansas to visit friends on the White River.
The crash was discovered after a resident reported hearing a low flying aircraft pass near his house followed by a loud thud. The crash was later discovered on nearby Davis Mountain.
The cause of the crash was unknown, but the area was covered by low-lying clouds at the time of the incident.
Officials reported that the plane was not a part of the search for missing campers from the Albert Pike Campground flooding located 15 miles away.

Joint Resolution Regarding Criminalization of Aviation Accidents

A joint resolution, agreed upon by several civil aviation safety organizations, has been released regarding the criminalization of aviation accidents. The agreed upon conclusion stated in this memo is that criminal investigations following aviation accidents can hinder the process of determining failed safety procedures due to an unwillingness of involved persons to provide information for fear of criminal prosecution
This resolution would allow the investigating agencies to better discern the causes of accidents by recieving more information from involved parties under the theory that people will more freely provide information when they are not threatened by criminal prosecution.
Another reason stated for the need of non-criminalization is the fact that there are already in place numerous civil options for the victims. These civil options are also seen as deterants for potentialy unsafe airlines because of the monetary fines involved in such actions.

Putnam County's Most Dangerous Intersection

The Intersection of Highway 111 and Quinland Lake Road is largely considered to be the most dangerous in Putnam county. The site has seen more accidents than any other site in Clarksville or Putnam County.
The latest accident involved an SUV attempting to cross Highway 111 onto Quinland Lake when in was struck by a truck. The SUV then collided with another vehicle at the intersection.
This accident sent two people to Cookville Regional Medical Center with non-life threatening injuries just days after another accident on the same spot injured two elderly people.
While the intersection is state owned and is scheduled for reworking by TDOT, the city of Algood is requesting that the process be sped up due to the danger to motorists.
The project, which is in the property acquisition phase, will completly rework the intersection as well as add traffic lights.
The need for the project is summed up by a statement given by a THP Trooper in which he affirms that he has been dispatched to Highway 111 and Quinland Lake Road "more than any other intersection."

The plan dictates the actions that the NTSB must take in the event of a large scale aviation disaster. This includes which agencies will be involved and to what extent.
This plan has been made available to the public and is a valuable tool in rating the performance of the NTSB during and after accident recovery and investigation. The layers will head committees focused on the two major categories of suits brought against Toyota. Two lawyers will head the wrongfull death suits brought on by unintended acceleration of the vehicles. The second committee will oversee the larger number of suits claiming a decrease in resale value of the cars after the acceleration malfuntion based recalls.
While the National Highway Traffic Safety Administration has already fined Toyota $16.4 million, the company has yet to admit any responsibility to its customers. 53 deaths have been attributed to the acceleration problem by the NHTSA.
The lawyers appointed to the case are preparing for a long battle with a well funded foe.

Bus Crash In Clarksville TN Injures Driver

A school bus loaded with thirty elementary students was involved in a crash in Clarksville, Tennessee.

The students were cleared by medical personnel and transferred to another school bus.

The driver of the other vehicle, who attends Clarksville High School, was taken to a local hospital and treated for minor injuries.

E-mails sent by a top U.S public relations official for Toyota five days before the companys massive recall show that company had previous knowledge of the accelerator pedal malfunction.
"We are not protecting our customers by keeping this quiet," wrote Irv Miller, group vice president for environment and public affairs. "The time to hide on this one is over."
Mr. Miller begins his e-mail by saying that their is a tendancy for mechanical failure in the accelerator systems built by a certain provider.
Other released e-mails show company executives ordering employees to keep quiet about the malfunction until they can come up with a remedy for the problem.

Medical Center Air Ambulance hospital Wing NTSB Crash Report

On March 25, 2010 a Eurocopter AS-350-B3, operated by Memphis Medical Center Air Ambulance Service, doing buisness as Hospital Wing, crashed near Brownsville Tennessee. The Pilot and two flight nurses were killed in the accident.
The helicopter departed Jackson-Madison County General Hospital Heliport en route to Haywood County EMS Heliport in Brownsville.
Before departing for the return trip the pilot recieved a phone call from the oncoming shift pilot saying he was concerned about the weather. However, the pilot did not want to leave the helicopter on the hospital's helipad.
The pilot then stated that he believed it would take 18 minutes to get the helicopter back to base. The pilot along with the two flight nurses then took off in an effort to beat the storm.
The oncoming pilot, who was watching and waiting for the helicopter's return, witnessed an "immediate" loud clap of thunder and lighting that made him jump. This lightning strike took place immediately after the oncoming pilot hung up the phone with one of the flight nurses that had just reported that they were 30 seconds out.
Worried about the lighting strike the oncoming pilot attempted to calll the flight nurse back unsuccessfuly. He then ran to contact the ambulance service located at the facility.
Three witnesses near the accident site reported thunder and lightning strikes at the time of the accident.
The helicopter was located two and a half miles from its home base in a field where it crashed and was destroyed by fire.
It was determined that the copter impacted the ground in nose-level, 40-degree left bank attitude causing all three blades to impact the ground and leaving two of the blades with broomstraw damage.
The helicopter, including the rotor blades, was examined for evidence of a lighting strike, with none found.
The last second communications and actions of the pilot are not known due to the absense of a cockpit voice recorder as well as a flight data recorder.

Carthage TN Auto Accident Injures Four

Four people were injured in a three-vehicle automobile accident that shut down the Carthage, Tennessee bypass.

Tennessee Highway Patrol Trooper Jimmy Tisdale, who investigated the crash stated that four people were traveling in the three vehicles and all were injured.

It was the investigating Tennessee State Trooper's opinion that the crash occurred when one of the three vehicles involved crossed into the west bound lane, striking the other two vehicles head on.

Toyota's former head lawyer alleges cover up.

Toyota’s former head lawyer, Dimitrios Biller, has filed a lawsuit against the company accusing it of conspiring to withhold evidence in rollover cases and then forcing him to resign.

Biller claims that Toyota had a database known as the, “Book of Knowledge,” that listed all of the design flaws as well as “countermeasures” used to resolve the problems.

Biller discovered the database while working on a case and claims to have warned Toyota that the database should be released during litigation but Toyota’s management refused.

Owners of Toyota and Lexis vehicles involved in the recall that have not been injured by the flaws in these vehicles may have overlooked the fact that they might have lost a large piece of their investment in their vehicle due to a drop of the fair market value related to the intentional concealment of these problems.

Hospital Wing Medical Helicopter Crash Near Brownsville TN Being Investigated – Other Life Flight Company Declined To Fly Due To Weather

Federal aviation officials are investigating the cause of the fatal crash of a Hospital Wing medical helicopter near Brownsville, Tennessee. Another medical helicopter company, AirEvac Lifetime turned down the flight due to dangerous and threatening weather.

Three people including the pilot and two nurses were killed in the crash of the Hospital Wing medical helicopter. They were Doug Phillips a retired Memphis Police Department pilot, Cindy Parker and Misty Brogdan, both flight nurses. Phillips and Parker were from Dyersburg, TN while Brogdon resided in Jackson, TN.

The National Transportation Safety Board (NTSB) and the Federal Aviation Administration (FAA) are investigating the crash that occurred in a wheat field about four miles east of Brownsville. Brownsville is approximately 55 miles northeast of Memphis, Tennessee.

Over a year ago, The National Transportation Safety Board (NTSB) held hearings on how to stem the "drastic increase" in medical helicopter accidents across the Country.

During a recent 12-month period, the board investigated nine fatal medical helicopter accidents that killed a total on 35 people.

The NTSB recommended to the Federal Aviation Administration (FAA) in January 2006 that all medical helicopter or chopper operators should be required to develop and implement a risk evaluation program. They further recommended that they be required to use dispatch and flight procedures that include up-to-date weather information. Another recommendation by the agency was to require the installation of "terrain awareness and warning systems" on the emergency aircraft. A final recommendation was to require medical flight operators to follow federal regulations regarding their flights.

The recommendations of the NTSB have not been fully implemented, the FAA as of 2010.

Toyota Problems Land in Tennessee

Toyota's recent problems with recalls due to uncontrollable acceleration have spured the filing of a class action lawsuit by attorney Keith Williams, of Lannom and Williams in Wilson County, who is representing a Lebanon owner of a 2009 Toyota Camry.
The lawsuit will not be for damages or injuries resulting from the vehicles, but the loss of resale value that the malfunctions have caused.
Additional plaintiffs may also join the case before the lawsuit is filed. According to Williams the lawsuit will ask the court to certify the case as a class action on behalf of all Toyota owners in Tennessee and appoint his client, the owner of a 2009 Toyota Camry, as the representative of the class.

Tractor Trailer Wreck on I-65 Outside Nashville Claims Trucker's Life

Ethiopian Air Flight 409 Tumbled Out of Sky On Take Off From Lebanon

The crash of Ethiopian Airlines Flight 409 that went down off the coast of Lebanon is still being investigated. Witnesses reported that the Boeing 737 tumbled like "fire falling down from the sky" into the Mediterranean Sea.

The Boeing 737 is considered one of the safest commercial aircrafts in service today, although it has been involved in a series of incidents and crashes over the past 15 years. These crashes have been linked to problems with a valve in the rudder assembly. The valve reportedly would malfunction and cause the rudder to turn independently of the pilot's commands.

The plane was carrying 90 passengers and crew bound for the Ethiopian capital of Addis Ababa. All aboard were killed.

Tennessee Nursing Homes Rank Fifth Worst in U.S.

Tennessee's nursing homes have ranked in the bottom five for the nation for the second year in a row. Out of a rating system of one to five stars, five being the best, more than 60 percent of Tennessee's 319 nursing homes got low ratings.

Toyota is building death traps and they know it. That is what an attorney for a Houston woman's family told reporters.

The woman's family filed a wrongful death lawsuit citing Toyota's alleged faulty accelerator pedals. This is the third such case filed in response to acceleration problems that prompted Toyota to recall millions of vehicles and halt some production.

Other lawsuits involving similar circumstances have been filed in California and Michigan.

Ethiopian Airline Flight 409 Flight Data Recorder Located

The Flight Data Recorder (also known as the "Black Box" or "FDR") of Ethiopian Airline Flight 409 that crashed off the coast of Lebanon, has been found in 1,300 meters of water in the Mediterranean Sea. The FDR has not yet been retrieved.

The plane crashed about 2 miles west of the town of Na'ameh, which is about 9 miles south of Beirut.

Four Fort Campbell soldiers died when their Black Hawk helicopter crashed during a training mission on Mount Massive in Colorado.

Ethiopian Airlines Flight 409 Crashes on Takeoff in Lebanon

Ethiopian Airlines Flight 409 crashed of the coast of Lebanon in the Mediterranean Sea with 90 people on board.

The Government-owned Ethiopian Airlines is one of the largest in Africa.

Toyota Recalls 2.3M Vehicles Because of Dangerous Accelerator Pedals

Toyota is recalling 2.3 million vehicles in the U.S. to fix accelerator pedals that can become stuck, the latest in a string of quality problems that have bedeviled the Japanese automaker.

Piper Crash at Memphis International on November 29, 2009 Pilot Error or Some Unknown Cause?

A pilot was fatally injured and three passengers received serious injuries when their plane crashed while landing at Memphis International Airport (MEM), Memphis, Tennessee on November 29, 2009. They were in a Piper PA-32RT-300, N9243C, which was substantially damaged during the crash.

The NTSB preliminary reports indicates that the aircraft was attempting an ILS landing on runway 18L but drifted off the localizer, left and too low for some unknown reason.

The preliminary NTSB report indicates pilot error or some unknown cause.

I-440 East On-Ramp in Nashville Causing More Accidents Than in Years Past

The steep curve at the I-440 on ramp from 40 eastbound in Davidson Co. has seen a sharp increase in accidents in the past five years.

The Metro police department and Tennessee Highway patrol have worked a total of 121 accidents on the curve in the past five years, 28 of which resulted in serious injuries.

These numbers are 10 times higher than five years ago.

Nashville Ambulance Driver Suffered Medical Crisis Before Fatal Wreck

The Nashville ambulance crash, that killed paramedic driving the ambulance and the 78 year old passenger Sue Bly , may have been caused by the preexisting medical conditions of the driver.

The ambulance driver, David Cline, had a history of vehicle accidents caused by seizures and a condition called narcolepsy which causes a person to fall asleep uncontrollably at any given time.

If an employer knows, of should have known of a dangerous medical condition of an employee, the employer may be held liable for “negligent entrustment.” If the employer is found to be negligent for entrusting a vehicle to an, ”at risk” employee, the employer may be help liable for damage caused by the employee.

Nashville Construction Worker Electrocuted Working in Clarksville Tennessee

An employee of DJ Drilling and Blasting of La Vergne was electrocuted while working at a Clarksville, TN construction worksite.

The man, who was not identified, was operating a drilling rig when the equipment came in to contact with a power line that caused his death.

He was later pronounced dead at Gateway Medical Center.

A car accident in Lebanon TN on I-40 near the Beckwith Road exit claimed the life of old Brian P. Ralls, 23 of Brentwood. The crash occurred when the vehicle driven by 19-year-old Cody L. Ingram of Lebanon crossed the median on I-40, and hit two vehicles going the other direction. The driver of the first vehicle hit was John H. Ray, 48, from IL who survived the crash.

The interstate was shut down due to the three car pile up.

A Cookeville woman was killed and two men were injured in a traffic crash near Sparta, TN. Amy D. Tollison, 32, of Benton Young Road, Cookeville, died in the one-vehicle accident, says a report by State Trooper Kevin Ballew. Daniel M. Garcia, 22, of Hickey Ridge Road, Baxter, and Justin F. Tindle, 20, of Massa Road, Baxter, were injured in the crash, the report says. It happened on Wildcat Road.

A MH-60 Blackhawk Helicopter crashed in Colorado killing four Fort Campbell-based Delta Company, 1st Battalion, 160^th Special Operations Aviation Regiment members. The soldiers were killed while conducting mountain and environmental training near Mount Massive. Those killed in Wednesday's crash were Chief Warrant Officers 4 Terrance Geer, 40, of Casper, Wyo.; and Robert Johnson, 41, of Seattle; and Staff Sargents Paul Jackson, 33, of Lancaster, Md., and Chad Tucker, 28, of Titusville, Fla.

Macon County, Lafayette, TN Motor Vehicle Accident

Connie Harrison and her son, Phillip Harrison, were both air lifted to Vanderbilt Hospital during the early morning hours of September 5th, after wrecking their 1997 Dodge Caravan off of Long Creek Rd. in Lafayette, TN.

Mrs. Harrison was behind the wheel of her 1997 Dodge Caravan traveling north on Long Creek Road with her son when she lost control of the vehicle driving off the right side of the road into a culvert. The van then proceeded through a neighboring lawn where it then overturned after hitting a tree and crashed through a fence before coming to rest on the vehicle's roof in an open field. Mr. Harrison, completely ejected from the van, managed to pull his mother from the vehicle after it had caught fire. According to Deputy Michael Veatch, upon arriving at the scene Mrs. Harrison was unresponsive, while Mr. Harrison was complaining of back and neck pain. Both were air lifted to Vanderbilt Hospital.

A Vanderbilt Hospital spokesman stated that Mrs. Harrison was in the trauma unit and was listed in critical condition, while her son had been released.

School Bus Accident Injures 21 Children Near Winchester TN

A minivan was struck by a pick up truck on Murfreesboro Road in South Nashville sending two adults and three children to the hospital. The minivan was attempting to turn into a church parking lot when the collision occurred. Although none of the injuries are life threatening, Metro Police Department said that the children in the minivan were not restrained properly and there was a booster seat in the back that appeared to be installed incorrectly.

Lebanon Native Dies in Car Accident

A single-car accident in the Norene community of Wilson County, TN on Septembe, claimed the life of a Middle Tennessee man. The crash occurred on Cainsville Road near Simmons Bluff Road. The victim, Kenneth W. Allen, 53, of Alexandria, was the only person in the vehicle according to a spokesperson for the Tennessee Department of Safety. Allen, a Lebanon native, was transported to Vanderbilt Medical Center where he died.

Albany International to Close Portland TN Plant

Albany International Corp. announced that it is closing its Portland, TN plant. The fabric plant closing will leave 156 Tennessee workers without a job. A final shutdown expected by June 30, 2010. Two families have retained B. Keith Williams to further investigate the circumstances surounding the near tragic incident of Southwest Flight 2294.

Southwest flight 2294 was traveling from Nashville, TN to Baltimore, MD when the passengers were forced to experience a life-changing event. The Boeing 737-300 made an emergence landing in Charleston, WV after an explosive and sudden decompression of the cabin while at 34,000 feet. This sudden cabin decompression was caused when the fuselage ripped open forming a 17 inch hole near the rear of the airplane.

Aria Airlines Flight 1525 Crashes Killing 16 - Injuring 21

Another Russian-made airliner, Aria Airlines flight 1525, skidded off the runway in Mashhad, Iran and caught fire as it made a hard landing. Sixteen people were killed in this Iranian commercial plane crash. Twenty One passengers were injured in the crash.

It was the nation's second deadly plane accident in two weeks. Caspian Flight 7908 crashed less than two weeks prior near Qazvin in North Eastern Iran, claiming 168 lives.

James Michael Bowers, 46, of Mt. Juliet , TN was flying a single engine airplane from Tennessee to Wisconsin when it crashed in Wabash County, IL.

Bowers was solo piloting a 1941 Luscombe vintage single-engine airplane to an experimental aircraft show in Oshkosh, WI. He was the only person on board and died upon impact.

Iranian Plane Crash Kills 16 Injures 21

Aria Airlines Flight 1525 from Tehran, slid off the runway during an emergency landing killing 16 people and injuring 21. The front end of the plane was sheared off and caught fire after the hard landing at the Mashhad airport in eastern Iran.

The aircraft was another Russian made aircraft making it the second Iranian commercial airline crash in two weeks. This points to the poor state of repair of the Iranian commercial airline fleet.

Caspian Airlines' flight 7908 "black boxes" have been recovered. The Russian made, Tupolev 154M jet crashed 16 minutes after takeoff in northern Iran while in route to Armenia. All 168 aboard were killed.

The so called "black boxes" include the aircraft's cockpit voice recorder (CVR) and the flight data recorders (FDR). The CVR and the FDRs will be key in determining the cause of the crash which remains unknown. CVRs and the FRDs are built to survive crashes and intense fires such as this one. They record a plane's performance, such as air speed and altitude, as well as communications between the cockpit crew and with air traffic controllers on the ground.

Plane Crash Kills All 168 On Board

A commercial airliner has crashed killing all 168 on board. The fatal crash occurred in Northern Iran when a Russian-made Tupolevs jet flying inder the name of Caspian Airlines was heading from Tehran to the Armenian capital Yerevan.   The 168 included 153 passengers and 15 crewmembers.

Caspian Airlines is a Russian-Iranian joint venture. The venture was started in 1993.

Iran has frequent plane crashes often because of bad maintenance of its aging aircraft.   In February 2006, a Russian-made TU-154 operated by Iran Airtour, which is affiliated with Iran's national carrier, crashed during landing in Tehran, killing 29 of the 148 people on board.

Medical Malpractice Verdict Sets Record in Memphis

A medical malpractice verdict was reached by a Shelby County, Tennessee jury is believed to be one of the largest verdicts in the state's history. The $24,000,000 judgment was to repay Courtney and Robert Hill for the damage that had been done to their lives due to the medical negligence of Dr. Claudia Moise, and her clinic, OB-GYN Specialists P.C. of Memphis.

The lawsuit was filed by the DeSoto County, Mississippi residents because the OB-GYN failed to diagnosis a lump Hall complained about over the course of 18 months. The lump turned out to be breast cancer.

Courtney Hill stated that all she hopes is that it will help physicians remember that further testing is required.

A commercial airline flight from Nashville, TN was forced to make an emergency landing after the cabin suddenly depressurized, deploying cabin oxygen masks. The Southwest Airlines, Boeing 737 was in route from Nashville to Baltimore and made the emergency landing in Charleston, WV after a football-sized hole formed in the rear of the fuselage forward of the vertical stabilizer. Cessna Airworthiness Directive issued by FAA 150s and 152s

The FAA has issued an airworthiness directive (AD) for all Cessna 150 and 152s. This AD affects some 17,000 Cessnas and was first proposed in 2007. It is reported 1,400 workers for the State of Tennessee will lose their jobs. Many hard working State of Tennessee workers are going to lose their jobs within the next 13 months.

The economy is affecting Tennessee workers in Nashville and Lebanon area. On May 28, 2009 the Tennessean newspaper reported that Governor Bredesen's plan to push layoffs back until next year has collapsed. The State plans to layoff 1,400 workers in the next 13 months. The state mental health sector is expected to take the hardest hit. Many hard working Tennesseans are losing their jobs. A plane crashed on approach in northeast Brazil on Friday May 23, 2009, killing all 11 people on board, according to cnn.com. Among the dead were 11 adults and 4 children.The airline crash occurred near the resort town of Trancoso, Brazil. It was reported that said it was raining, but visibility was not bad.

The plane had taken off from Sao Paulo. It was also reported that there was no chance of survival at the fiery crash site.

The plane belonged to Roger Wright a prominent businessman -- who was traveling with his wife and other family members. Wright, 56, was an ex-partner of Banco Garantia and owner of Arsenal Investimentos.

Garage Door Causes Death of Child Alleged in Lawsuit

A faulty garage door caused the death of a 6 year old child when it closed on him, according to a lawsuit filed by his parents. The lawsuit alleges was filed against the homeowner, the installer of the garage door and the manufacturer of the garage door. The minor's death was due to compressional asphyxia after he was trapped under the garage door, according to the Cook County, IL's medical examiner's office. The young child had been playing outside by himself when his 9 year old brother found him fatally injured.

Job Loses May Be Coming For Tennessee Nissan Workers

Nissan, one of Tennessee's leading automotive employers, reported a $2.4 billion annual net loss on Tuesday. Nissan has two plants in Tennessee the assembly plant in Smyrna and engine plant in Decherd. With this the first yearly loss in a decade Nissan is looking to cut 20,000 jobs. Last year, Nissan had early termination that resulted in a reduction of nearly 1,200 jobs.

Motorcycle Accident Deaths Rise in Tennessee

Motorcycle fatalities in Tennessee have increased three fold in the past 10 years. In 2008, 143 people were fatally injured in motorcycle accidents. Just 10 years earlier in 1998 the number of deaths due to motorcycle crashes was 42. In response to this 340% increase in fatal motorcycle accidents, the Tennessee Highway Patrol has launched a Share the Road campaign to remind drivers to watch out for motorcyclists.

Motorcycle Crash Kills Lebanon TN Police Officer

Lebanon TN police officer Jeff Stone was killed in a motorcycle crash Saturday night. The accident happened at the intersection of Highway 231 / South Cumberland Ave. and Jennings Avenue. Stone was off duty while traveling South on US 231 on his 2006 Harley Davidson, when a 1997 BMW pulled out in front of him causing Stone to strike the BMW is the drivers door. Stone was declared dead at the scene.

The federal investigation into the crash of Continental Connection Flight 3407 near Buffalo, New York is focusing in on an important potential shortcoming on the part of Colgan Air, which operated the Continental Connection aircraft. The National Transportation Safety Board's investigation is looking into Colgan's pilot training program, especially regarding how the plane's stall protection system operates in icing conditions.

Continental 3407 Crash Near Buffalo Gets NTSB Hearing

Continental Connection Flight 3407 will receive a public hearing by the NTSB on May 12-14, 2009.

The hearing will cover the safety issues concerning the crash of the Bombardier Dash 8-Q400 that occurred on February 12, 2009 near Clarence, NY. Issues covered in the hearing will include: icing effects of the plane’s performance, cold weather operations, sterile cockpit rules, crew experience, fatigue management and stall recovery management.

The purpose of the hearing is to develop facts the appropriate facts for the investigation of the worst transportation tragedy in the US in 7 years.

On March 24, 2009 TRW laid off 42 employees in Lebanon, Tennessee and 11 employees in Nashville, Tennessee. Automotive related job loses are at a record high for the Middle Tennessee area. Questions Surrounding Responsibility For Death Of 7 Year Old School Boy In Warren County

The recent death of a 7 year old Warren County boy while crossing the road to catch his school bus raises a number of questions in the aftermath of this terrible tragedy. The bus driver has now reportedly been indicted in connection with the death, and allegedly knew her bus lights were not working. In situations like this, the question arises who will be responsible for the loss? Was it the other driver? Was it the bus driver? Was it the school system itself? And if the school system shares some responsibility, can it be liable under the Tennessee Government Tort Liability Act, which allows recovery in only certain types of situations with a limited amount that can be recovered? Even if there is a general limit under the Tennessee Government Tort Liability Act, did the school system fail to properly train its drivers or was there some policy in place that led to this accident? If so, the school system might be subject to a federal civil rights action, which would not be subject to the liability limits of the Tennessee Government Tort Liability Act. These are all questions that will be answered with time in this matter. For now, our prayers stay with the family as it tries to cope with the loss of their little boy.

Ten students and the driver of a White County, Tennessee school bus were injured after their bus crashed into a tractor trailer on April 29, 2009. The school bus was carrying 28 students on McMinnville Highway near Sunset Drive, when the bus crashed into the semi truck. Cookeville Woman Fatally Injured in I-40 Crash

Catastrophic failure of the main rotor gearbox of the Super Puma Helicopter was the cause for that crashed into the North Sea off the coast of Scotland earlier this month. The Consumer Product Safety Commission (CPSC) announced a major recall of over 145,000 Yamaha Rhino off-highway recreational vehicles. The CPSC states that consumers should immediately stop using these ATVs. Yamaha Motor Corp. agreed to a free repair program to address the safety issues with all Rhino 450, 660, and 700 models. Ambulance Crashes Into TDOT Dump Truck on I-24

A Coffee County ambulance crashed into a TDOT dump truck on closing I-24 for a short time.

The Saturday Independent reported, that there were multiple injuries in the accident that bottled-necked I-24 - flooding the streets of Manchester with interstate traffic. One person involved was airlifted to Centennial Medical Center in Nashville with non life threatening injuries. One of the ambulance workers involved was taken to Medical Center of Manchester.

The accident occurred near mile marker 114 and the Highway 41 South exit in Manchester, according to wkrn.com.

Victor Jones, with the Manchester Police Department said, the injured worker had just gotten up from the bench on the back passenger side when the ambulance hit the trailer and truck, peeling back its entire right side.

Robinson Helicopter's R22 and R44 models main rotor blade delaminations have become the subject of Federal Aviation Administration's (FAA) Airworthiness Directive (AD) 2007-26-12, superseding the Company's Service Bulletin.

The AD states that main rotor blade skins begin to delaminate, debond, or separate the ski near the blade tip. This delamination or debonding can occur when the bond line is exposed due to excessive erosion of the blade finish, or when corrosion occurs on the internal aluminum tip cap. Read more...

On Monday a construction worker was killed by electrocution while working on a church expansion project in Wilson County Gladville, Tennessee which is just east of Nashville.

With the Wilson Emergency Management Agency Station next door to the church and with immediate response the man's live was unable to be saved.

Deadly Crash Closes I-24 North of Nashville Toward Clarksville

A deadly auto crash, North of Nashville closed Interstate 24 on March 24, 2009.

The fatal wreck happened near exit 35 outside of Joelton. It involved a husband and wife from Iowa who were traveling through Tennessee in their Nissan Altima. They collided with a Chevy Trailblazer around 6:30 a.m.

News Channel 5 reports that one of the vehicles was traveling the wrong way down the interstate but at the time of the report it was not clear which vehicle was headed in the wrong direction.

The impact killed the Iowa wife and severely injured the Husband and the driver of the Trailblazer. Metro Police stated that the driver of the Trailblazer might have been drinking and driving.

The Tennessee Department of Transportation detoured West bound traffic off and around I- 24 West while the scene was cleared. Read more ...

Twelve (12) and Fifteen (15) passenger vans are used by various groups and organizations due to their ability to transport a large amount of people and their accompanying cargo. These dangerous vans are used as church vans, hotel shuttles, daycare busses, airport shuttles, school buses as well as many other uses. A fatal motorcycle crash outside Lebanon in Wilson County, TN claimed the life of a Nashville man.

Although it occurred late on St Patrick's Day, investigators did not know if alcohol played a factor in the incident.

A trooper for the Tennessee Highway Patrol said that the motorcycle wreck occurred when the driver lost control, crashing on Leeville Pike near Tuckers Gap Road. The intersection is very near where the Interstates of 40 and 840 intersect.

Tennessee aviation attorney B. Keith Williams filed a lawsuit in federal court on behalf of the widow and children of Roc Abou-Sakher, the pilot killed in the March 11, 2011 plane crash in Smyrna, TN.
The lawsuit alleges Mr. Abou-Sakher, a commercial pilot, was on a maintenance flight when the Garmin GPS and the S-Tec autopilot systems had an electrical conflict causing the plane to go into an uncontrollable nosedive resulting in Mr. Abou-Sakher’s death.
The suit also alleges that Smyrna Air Center negligently installed the avionics system in the plane.

Weehoo Bike Trailers Recalled Due to Fall and Crash Hazards

Weehoo Inc. of Golden, Colorado has recalled around 2,700 of their iGo bicycle trailers manufactured from April to July in 2011. The receiver on the trailer pulled behind the bicycle can break, leading to the trailer coming detached, potentially causing the rider to fall or crash. These bicycles were sold in most bicycle retail stores nationwide between April and July for around $390. So far, Weehoo has received one report of this incident, although no injuries have been reported. Consumers should stop use of the bicycle immediately and contact Weehoo for a replacement part to be installed on the receiver of the trailer.

A motorcycle crash kills a Lebanon, Tennessee woman and injures another in White County on Highway 84 near the England's Cove area. According the State Trooper at the scene, the couple was riding together when the motorcycle struck the guardrail while going around a curve, throwing the passenger off. The motorcycle and the driver came to rest further down the road. The passenger was pronounced dead at the scene and the driver severely injured. The report notes both riders were wearing helmets.

An auto accident involving an ambulance in Erin, Tennessee sent two people to Vanderbilt Medical Center by Air Ambulance. The crash occurred on Highway 49 in Houston County. State Troopers and Houston County officials responded to the scene. The ambulance and car were the only vehicles involved in the crash. The cause of the accident is still being investigated, although, officials have released that the ambulance driver was not at fault. The ambulance was not carrying any patients at the time but it is unclear whether the emergency lights were on.

Metro police responded to a motorcycle wreck that left a man critically injured. The local Tennessean identified as, 25-year-old Paul Rose was riding his motorcycle in South Nashville when he hit a truck. The truck was pulling out of a residence when the driver failed to yield. Rose was rushed to Vanderbilt Medical Center in critical condition. The driver and passenger in the truck was not injured.

Hoeganaes Plant In Gallatin, Tennessee Faces Hefty Fines After Third Fire In 2011

The Hoeganaes plant in Gallatin, Tennessee is dealing with their third fire in 2011. The previous fires have killed two workers and left a third worker critically injured. The latest fire involved five workers, three of which are in critical condition. The plant produces atomized iron powder for industrial customers and handles flammable combustable metal dust. Hoeganaes faces fines equaling up $42,000. The largest fine included was for $7,000 because of 4 inches of iron dust on equipment and floors. Hoeganaes has 20 days to respond to these fines. A four-person team is investigating from the U.S. Chemical Safety Board. The CSB has concluded that the past two fires involved the flammable metal dust that exploded killing two of the workers. Investigators are not linking the third fire to the other two at this time but there is an ongoing investigation.

Teenage Car Wreck Victim Awarded $4.4 Million In Damages.

The victim of a car wreck was awarded $4.4 million in damages in a negligence suit. The suit was filed in Greenville County Court of Common Pleas in South Carolina after a SUV struck a light pole during a rainstorm. Defendant, Duke Power Company, had designed the poles to shut down after being struck. The vehicle hit three light poles, two of which shut down but the third one didn't sending voltage through a young boy's body. The young boy, Xavier Massey, sustained only 4% burns on his whole body but has to have five surgeries. After the accident, Xavier has had to have several toes amputated due to the electriction. Xavier's family was faced to pay all of his medical bills reaching up to $600,000. Judge Robin Stilwell left the verdict up to the jury and the family was awarded $4.4 million in damages.

Indonesian Plane Crash Leaves No Survivors

A small passenger plane crash in Indonesia claimed the lives of everyone on board. The aircraft was carrying 27 people during a storm when it was preparing to make an emergency landing in Kaimana. The plane did not make it to the small town and crashed in the sea into the eastern province of West Papua. The plane was operated by the state owned Merpati Nusantara Airlines. The Indonesian archipelago relies on air transportation but has one of Asia's worst safety records. Merpati is one of the numerous Indonesian airlines that the European Union has banned from operating in the European Airspace.

Disney Princess Tricycles Recalled For Laceration Hazard

Disney Princess Tricycles are being recalled by their manufacturer, Kiddieland Toys Limited. The voluntary recall involved over 9,000 tricycles in the United States and Canada. The bikes are a hazard due to the castle and princess figurines that are on the tricycles handle bars. A child can hurt themselves if they fall on the handlebars. There have been only three reports so far due to the handlebars causing facial lacerations. The tricycles were sold from January 2009 through April 2011 at Target, J.C. Penney, Meijer and H.E.B stores nationwide and online. Consumers may contact the store to get replacement handle bars with the castle and princess' enclosed in a revolving dome.

The small aircraft was being flown by the production crew of the reality television show, "Campus PD". The show is based on crimes and disturbances that happen on campus. It is unknown as to what the television crew was shooting when the helicopter lost control causing it to crash into the back of an apartment building. The aircraft landed upside down in a driveway and the tail was bent at a 45° angle. There were four passengers onboard the helicopter, three of which are in the trauma unit at the local hospital and the fourth only suffered minor injuries. The Federal Aviation Administration is investigating the cause of the crash.

FDA Recalls Teva Acid Reflux Mediciation

The Food and Drug Administration has recalled the generic mediation called lansoprazole. The medication is a generic for Prevacid that is taken to reduce acid reflux and disintegrates when added to water. The FDA has released that any patients that are consuming this medication through a syringe or feeding tube need to stop immediately. The drug is an orally disintegrating tablet (ODT) that does not disintergrate completely and can clog up syringes or feeding tubes. The medication is manufactured by Teva Pharmaceutical Industries Ltd. There have not been any reported injuries. The FDA is investigating the issue and is reviewing the drug.

A plane crash in Knoxville, Tennessee has left authorites fishing a plane out of the Tennessee River. The Cessna 150 flown by Michael Matvy was flying west of Island Home Airport when the small aircraft lost all power. The pilot immediately notified officials after losing power to let them know that he had to make an emergency landing. After guiding the plane into the Tennessee River, local boaters reached Matvy who was hanging onto the plane's wing. Local authorities reached the scene quickly to make sure no one was injured. The Cessna 150 sank some 16 feet into the main channel. The cause of the plane losing power is still under investigation.

Southwest Airlines Grounds 2 More Boeings 737 300's Due To Cracks

Southwest airlines have grounded three planes so far after finding subsurface cracks resembling those that caused a Boeing 737 300 to make an emergency landing. Southwest Airlines Flight 812 landing was due to the fuselage rupturing causing a five-foot long crack on top of the plane. The cracks found were in two lines of riveted joints that run the length of the plane. Since Flight 812, Southwest has inspected 79 other Boeing 737 300s and they have all passed the inspection; 60 other aircrafts are going to be inspected. The National Transportation Safety Board has established a bulletin for all of the 737 300 models to compare to Flight 812 which was 15 years old and had about 39,000 takeoff and landing cycles.The bulletin insists on taking thorough inspections on the lap joints by the fuselage. The inspection is not mandatory now but the Federal Aviation Administration is likely to enforce the inspection. There are over 900 Boeing 737 300s worldwide and 288 of which are in the United States. An inspection in March 2010 found 10 different places of cracking in the plane's frame and 11 places where the stringer clips were cracked all of which were repaired or replaced. The FAA has stated general inspections were met prior to Flight 812 taking off.

Southwest Airlines Flight 812 Boeing 737 300 had to make an emergency landing at a military base in Yuma, Arizona. The plane was enroute from Phoenix, AZ to Sacramento, CA. A flight attendant was the first to notice the widespreading cracks that were surfacing on top of the plane. The flight attendant had stated, "All you could see was blue sky and sunshine when you looked up." A National Transportation Safety Board member said that the Boeing 737 300 was cruising at 36,000 feet in the sky and it dropped 11,000 feet in 4½ minutes. The crack was 5 feet long and 1 foot wide but not something that could have been noticed in the routine inspection in March 2010. Southwest Airlines has grounded 79 of its planes to do an inspection to insure there are not anymore cracks. The NTSB, the Federal Aviation Administration, Boeing, and Southwest are working together to insure that this does not happen again. Southwest mechanics cut off pieces of the crack and took it to the NTSB laboratory in Washington. The cause of these cracks is being investigated.

U.S. Marine Helicopter in Hawaii Crashes Claiming 1 Life and Injuring 3

A CH-53D Sea Stallion helicopter crashed claiming one life and injuring three others. After having to make an emergency landing, it landed in shallow waters in Kaneohe Bay, Hawaii. The military has released that two of the Marines are in critical condition and the third is in stable condition. The Life flight helicopter was transporting a teenager that had been in a car wreck when they ran into inclement weather. The National Transportation Safety Board has released that the main reason for the crash was due to the pilot’s decision and misjudgment of the weather conditions. 18 Wheeler Crash on I-24 In Rutherford County, Tennessee Claims Life.

A Murfreesboro, TN man was killed when his box truck collided into the back of an 18-wheeler on I-24. The Rutherford County man was pinned inside his vehicle when rescuers were trying to get to him. The Rutherford County Emergency Medical Service spokesperson, Randy White, commented “our guys were very heroic on that call. We just couldn’t get to him.” EMS could not release the name of the victim at this time. Traffic along I-24 was backed up for several hours while Sheriff’s deputies and Tennessee Highway Patrol cleared the scene. It’s said that the cause of the wreck was due to hydroplaning. During inclement weather, it’s more likely for these kinds of wrecks to occur.

Snow storm leads to wrecks across Middle Tennessee

The state of Maryland has sued the federal government over the crash of a state police helicopter that killed four people in 2008.

The lawsuit alleges that two air traffic controllers failed to respond properly to the pilots distress calls and the failure of the Federal Aviation Administration (FAA) air traffic controllers caused or contributed to the helicopter crash.

The lawsuit further claims that the FAA air traffiac controllers failed to provide adequate weather information and did not assist the pilot with the approach to the landing site while in heavy fog. Attorneys for the State argue that the FAA air traffic controllers were distracted by less important matters.

Damages in the amount of $4 million are requested in the lawsuit.

I-40 Truck Crash Proves Fatal

A truck crash on I-40 in Cumberland County, Tn resulted in the death of a North Carolina man.
The victim, who was driving 2007 Freightliner van, ran into the back of a 2006 International truck that had stopped or slowed due to a traffic backup caused by a previous wreck.
The driver of the Freightliner was injured, but it is not known how severely. Both drivers had been wearing their seatbelts at the time of the crash.

AirTran Flight 619 was reportedly forced to land in Memphis, Tennessee that departed from New Orleans because of smoke. The plane was headed to Milwaukee, Wisconsin when an odor and smoke was noticed in the cabin. The Boeing 717 landed safely, reaccommodated the passengers and continued on to Milwaukee.

Bitrax Recalls Child Safety Seats

Bitrax has recalled nearly 23,000 of their Chaperone Infant car seats. The seats were voluntarily recalled by the company due to a risk of laceration and choking attributed to a faulty piece on the chest harness. The small plastic piece is sharp and a potential choking hazard once broken.
The U.S. Consumer Product Safety Commission announced the product safety recall, which affects approximately 23,000 units, on Thursday, November 4, 2010.
The company recieved reports of three kids apparently sustaining minor lacerations and scrapes on their arms and a finger. The company also received one report of an infant sticking the small clip in his mouth.
This recall involves Chaperone infant car seats with model numbers E9L95P2 (Red Mill), E9L95P3, E9L95P5 (Cowmooflage), E9L69N9 (Moonstone) manufactured between April 2009 and May 2010.
Consumers have been urged to contact Britax to obtain a complimentary repair kit, which includes a replacement chest clip.

Medicare Patients Suffer From "Adverse Events"

A new study conducted by the government has found that an estimated 15,000 Medicare patients die each month from the care that they recieve in hospitals. Philips Professional Luminaires has announced that it will be shutting down it's White County, Tennessee plant on March 31, 2012. Local White, County residents better know the plant as Thomas Industries. Lafayette, Tennessee Wreck Claims the Life of Two

Two people were left dead in an auto accident that occurred outside Lafayette, Tennessee. A 2000 Toyota Camry driven by Thomas King of Macon County, Tennessee was traveling west bound when a 1995 Honda Accord driven by Kenneth Brandy of Scottsville, Kentucky collided head on. Both drivers died instantly upon impact. The cause of this fatal crash is still being investigated.

An Antarctic helicopter crash claimed the lives of four French citizens. Due to inclement weather, rescuers could not get to the crash site for two days. The distress beacon on the French aircraft was activated, and assisted the United States and French Air force to confirm the location. Plane Crash in Kossuth, Mississippi

A plane crashed while flying over Alcorn County, Mississippi on Tuesday during a storm in the mid south. It was reported by the Alcorn County Sheriff's Department that two people died in the crash. The plane left debris on a farm close to Kossuth while on its way to the Atlanta area.

Graco Quattro Tour and MetorLite strollers are being recalled because of the ability to trap small children who are not correctly harnessed in the stroller.

The recalled strollers are some older models which include Quattro Tour strollers made before November 2006, and the MetroLite strollers made before July 2007. Other than the Graco Quattro Tour and MetroLite Strollers no other strollers are being recalled. Graco is providing free repair kits for these strollers.

Consumers who have purchased either of the strollers mentioned above should discontinue using them right away until receiving the repair kit.

Evenflo Booster Seat Recalled Due to Danger

Evenflo booster seats are unsafe when involved in a high impact, frontal collision motor vehicle accident according to recent tests. Booster seats that have a model number beginning with "310" are considered dangerous, and the booster seats built between November 2009 and April 2010 have been recalled.

According to test if Children weigh more than 40 pounds they should not use the booster unless they receive the free repair bracket, however, they may use the seat as a belt- positioning booster until the bracket arrives.

Dangerous bathtub toys recalled by Munchkin, Inc.

Munchkin, Inc. has recalled approximately 34,000 submarine bath toys according to the U.S. Consumer Product Safety Commission. The recall is due to the of the intake valve, which sucks in water to propel the submarine, has the potential to suck to a child's loose skin and cause a laceration. There are 19 known cases of lacerations to boys' genital areas.

These bath toys were sold in retail and children's stores from November 2009 through September 2010. Consumers should contact Munchkin, Inc. in order to return the toy and obtain a replacement.

Macon County Man Killed in Vehicle Accident

A Macon County man was killed in an early morning car crash on Highway 10 South in Trousdale County. The 31-year-old man was a passenger in a 1999 Plymouth Neon driven by another Macon County man. The car veered of the road and struck a tree at around 2:30 a.m. Both men were found not to be wearing their seatbelts. The passenger was taken to Trousdale County Medical Center where he was pronounced dead on arrival. The driver was eventually transfered to Vanderbilt for treatment.

Paris Tennessee Amphibious Plane Goes Down In Kentucky Lake Near Paris

A Paris Tennessee plane crashed into Kentucky Lake near Paris Landing while attempting a training landing. The Savannah VG Light Sport aircraft was equipped with amphibious landing gear was built by Sky Kits.

Eric Giles who was in his first hour of training to earn his airplane single sea rating was piloting the plane when one of the floats cracked at the seam as it made contact with the water, causing the aircraft to overturn. Ernest Jiark of Murray, KY was giving Giles flight instructions at the time.

NES Worker Electrocuted

The man was working on a power line on Sones River Rd at the time of the incident.
NES is calling the death an accident, but would not provide details on how it happened.

California Horse Breeder Dies in Plane Crash

Prominent Southern California horse breeder Frank Vessels was killed when the twin engine Aero Commander Model 500-B he was traveling in crashed 80 miles south of Burns, Oregon.
Vessels, who was piloting the plane, was traveling from Redding, Calif., to Montana when the plane broke up in the air and was destroyed after crashing.
There was one passenger on board who was also killed in the crash.
The FAA and NTSB are investigating the cause of the crash.

Wreck Kills Mt. Juliet Teen

A sixteen year old girl was killed in a single vehicle accident in Mt. Juliet, Tn.
She was the passenger in a 2003 Mercury Mountaineer that was driven by a seventeen year old female.
The vehicle crossed the center line and then overcorrected, sending them off the road near 1120 N. Greenhill Road in Mt. Juliet. This caused the passenger to be ejected from the vehicle.
The driver sustained injuries while the passenger was later pronounced dead at Vanderbilt Children's Hospital in Nashville.
It was revealed that the injured driver had been wearing her seatbelt while the victim was not.
No charges had been filed at the time of this story.

Two Injured in Head-On Collision with Semi

Two teens were injured in Overton County after their Ford Ranger crossed the center line and struck a semi truck.
One of the teens was taken to Livingston Regional Medical Center, while the other was airflighted to UT Knoxville. Both are in stable condition with some broken bones.
The driver of the semi was not injured.
What caused the teen to cross the center line is not known.

Pilot Killed in Georgia Plane Crash

The pilot of a Piper PA 32 was killed when the small airplane crashed enroute from Madison County Executive Airport in Huntsville, Alabama to Mount Pleasant Regional Airport in Mount Pleasant, South Carolina.
The pilot was the only one on board the plane, which was registered to MS Saratoga LLC of Mount Pleasant, South Carolina.
The Georgia Bureau of Investigation has investigated the crash site and is now waiting on officials from the FAA and NTSB to process the scene.

A Beechcraft Baron BE58 twin engine plane crashed into a house killing both men on board.
The lone occupant of the residence escaped with his dog as both the plane and the house were engulfed by flames.
The plane, which took off from Arnold Palmer Regional Airport outside Latrobe, PA, crashed 11 minutes afer take off about 50 miles from the airport.
The pilot was said to have just bought the plane, and was flying with an instructor on board.
The FAA and NTSB are investigating.

Metro Police Captain Hit by Drunk Driver on I-24

A Metro police captain was hit by another vehicle while sitting in his car at a road construction site.
The woman driving the car never attempted to swerve or apply the brakes in an attempt to avoid the captain's vehicle which was stationary with it's overhead lights on.
The woman was pinned in her vehicle, talking on her cell phone, until rescue crews could free her. When asked about the accident by officers she was uncooperative.
Her demeanor at the scene and failure to avoid the captain's vehicle led officers at the scene to suspect that she was driving under the influence.
Both parties were taken to Vanderbilt Medical Center where the woman is listed as stable with serious injuries. The metro captain was being examined due to back and neck pain and is also listed as in stable condition.

Four Dead, One Injured in Michigan Plane Crash

A twin engine Beechcraft Model 58 crashed onto I-75 in Michigan killing the pilot, who was the 73 year old grandfather of the four children on board, three of which also died in the crash.
The thirteen year old grandson of the man was the only survivor of the crash and is still in the hospital with severe burns.
The plane crashed shortly after takeoff during what was supposed to be a short day trip to Mackinac Island in northern Michigan.
The grandchildren had been visiting from Isreal and had been in the States for about a week when the crash took the lives of the three girls and their grandfather.
The cause of the crash is still being investigated by the FAA and NTSB.

Fatal Plane Crash in Tulsa, OK

A twin engine Cesna airplane crashed in a heavily wooded area about one mile north of the Tulsa International Airport. Two of the occupants died in the crash while another died a short time later at nearby St. John Medical Center.
The plane, which was en route from Pontiac, Michigan to Jones Riverside Airport in Jenks, asked for permission to land at Tulsa International citing low fuel.

Fatal Motorcycle Accident in Macon County

A July 4th motorcycle accident in Macon County took the life of a Trousdale County man.
The man, who was riding a Harley Davidson motorcycle, collided with a late model suv carrying a family of four when the suv made a left turn infront of the oncoming motorcycle at the intersection of Highway 52 West and Oakdale Road.
The man riding the motorcycle was from Hartsville, TN and died enroute to Macon General Hospital.
The family in the suv were shaken but unharmed in the accident.

Cookville, TN Woman Injured in Violent Crash

A Cookville, TN woman was injured when her Suzuki Vitara left the road at high speed near Knights Inn.
The vehicle struck a utility pole and broke through a fence before hitting a ditch which caused it to flip and land on it's side. The woman was ejected from the vehicle at some point during the crash.
Paramedics treated her at the scene before she was airlifted to Erlanger Hospital in Chattanooga, TN.
Officers investigating the scene stated that speed was definately a factor in the crash and that it was "lengthy and violent."

Fatal Head-on Collision in Madison, TN

A woman was killed on Myatt drive in Nashville when she drove into oncoming traffic and struck another vehicle head-on.
One vehicle was able to avoid the woman's vehicle traveling the wrong direction in their lane only to witness her vehicle crash into the car behind them.
The woman was pronounced dead at the scene while the driver of the vehicle she hit was sent to the hospital with severe injuries to his lower body.
The witnesses in the vehicle that was able to avoid the crash stated that it was like "she was playing chicken with us."

MTA Bus Hits Elderly Woman in Crosswalk, Nashville, TN.

An elderly woman was injured when she was hit by an MTA bus while crossing the street in Nashville, TN.
The 72 year old suffered serious head trauma and remains hospitalized in critical condition at Vanderbilt Medical Center.
The MTA bus was turning onto Charlotte ave from 4th ave struck her at that intersection.
Neighbors say that the woman could be seen walking to the store on a regular basis from her home at the James Robertson apartments.
It was unclear at the time what color the lights were when the woman was hit and if the bus driver ran light. The driver has been placed on routine leave pending an investigation.
Vanderbilt has been attempting to contact the family of the woman.

Church Van Crashes, Injures 14

The driver of a church van, carrying 14 members of The New Vision Ministries from White Pine, Tn, sped over a railroad crossing and struck an embankment after running a stop sign.
Twelve of the passengers were taken to Morristown-Hamblen Healthcare System while two others were airlifted to U.T. Medical Center Knoxville.
Police cited the driver Cecil Garret for running a stop sign, driving on an expired registration and not wearing a seatbelt.
Most of the teen-aged victims were treated and released, but at least three have more serious injuries and are still in the hospital.

Deadly plane crash in Howard Co. Arkansas

Two couples traveling to the White River in Arkansas were killed Sunday when their 1977 Piper Fixed Wing single engine went down in Howard County Arkansas. Carl and Carolyn Davis, as well as Mike and Terri Hunt took off from DeQueen Arkansas and were on their way to visit friends on the river when the plane crashed.
The crash was reported by resident near the Davis Mountain crash site that reported hearing a low flying aircraft followed by a loud thud.
Authorities said the plane was not involved in the search for the missing campers of the Albert Pike Campground flooding which was only 15 miles from the crash site.

Mangalore crash to be investigated by a court of inquiry

The crash of Air India Express flight IX-812 in Bangalore India is set to be investigated by a court of inquiry.
The Boeing 737-800 crashed when it overshot the runway at Bajpe Airport on May 22, in Bangalore India. The plane plunged down a hill and into a ravine where it burst into flames killing 158 people on board.
The government has appointed its retired Air Marshal to conduct the investigation as well as four experts from different fields to assist him.

Toy Dart Gun Sets Recalled After Boy Dies

A recall has been issued for toy dart sets after children die from choking. The older children, 9, 10, died after choking on the darts tip.
Federal regulators issued the recall after the deaths.

For the family of Kentrell Rodgers, a 9-year-old West Side Chicago boy who died in August 2007, the recall came too late. Kentrell put the small toy dart in his mouth and accidentally inhaled the product, a Chinese-made toy imported by Henry Gordy International Inc. and sold at Family Dollar stores.

It seems that the death of Kentrell Rodgers could have been prevented if the company had taken action following the death of another boy, Desman Young, in 2006

Henry Gordy also had a chance to recall the toy before Kentrell died. The company notified its insurer about Desman's death in January 2007, according to a court records.

"If the company would've been straightforward after this happened to Desman, there never would've been the Illinois death," said Jay Urban, an attorney for Desman's family.

Lawsuits are pending.

Two more people were injured during a three car wreck at Putnam county's most dangerous intersection.
The intersection of Highway 111 and Quinland Lake Road in Algood was the site of yet another accident in which a small SUV attempted to cross Highway 111 and turned into the path of a truck that struck the SUV. The SUV then collided with another vehicle.
This is just the latest in an ongoing string of accidents at this site. The most recent being a few days before when two elderly people were injured.
TDOT is planning to rework the intersection with traffic lights but the project is still in the property acqusition phase. While the intersection is considered a state road the city of Algood has been pushing for the completion of the project due to the high number of accidents at the site.
The danger to motorists at the intersection is higher than anywhere else in the county as evidenced by the statement of a THP Trooper that said he has been dispached to the intersection of Highway 111 and Quinland Lake Road "more than any other intersection."

Cookeville Tractor Trailer Crash Injures Two on I-40

A automobile accident on I - 40 near Cookeville, TN seriously injured two men when their tractor trailer hit a bridge pillar. The wreck happened in the eastbound lanes of Interstate 40 near the 288 exit.

The two injured men were transported by medical helicopter to Erlanger Medical Center. Both occupants of the truck were trapped in the wreckage and was execrated by the Cookeville Fire Department and Putnam County, Ambulance Service.

The tractor trailer involved, a Volvo which belongs to D&S Transportation Inc. of Decatur, Alabama, was hauling U.S. mail, and that cargo spilled out and was strewn around the scene.

A stretch of I-40 in the area was closed to traffic for a time, and vehicles headed that way were re-routed through Cookeville to bypass the accident scene, where two helicopter ambulances had to land.

Vietnam Veterans Boulevard Five Car Pileup Injures Three

Three people were Injured in a five car pileup on Vietnam Veterans Boulevard near the Highway 109 and Highway 25 intersection . The Injured were transported to Vanderbilt University Medical Center in critical but stable condition.

The Tennessee Highway Patrol reported that a Peterbilt tractor trailer was traveling northbound on Highway 109. While approaching the intersection of Highway 25, it hit the brakes too late and crashed into a Nissan Altima causing a chain reaction involving three more vehicles.

Four people in the Altima were injured. Passengers Derrick Gilbert, Linda Williams and Annette Adams were all transported to Vanderbilt University Medical Center in critical but stable condition. The driver, Ernest Britton, of Gallatin, was treated and released from Sumner Regional Medical Center Tuesday night.

According to THP, The driver of the Peterbilt, 75-year-old Robert Browning, of Hamilton, Ohio, was uninjured.

Other vehicles involved in the wreck were another tractor trailer, a 1991 GMC Sierra and a 2005 Toyota Highlander that was pulling a U-HAUL trailer. The drivers of all of these vehicles were injured.

Air India Express Plane Crash Kills Scores

Air India Express flight 812 from Dubai to Mangalore in Southern India, overshot the hilltop runway where it was landing killing over 160 passengers. The crash happened when the Boeing 737 traveled off the end of the runway.   The runway is located on a hilltop with valleys of 200 to 250ft immediately beyond paved surface of Runway. Mangalore-Bajpe Airport (IXE). Investigators on the scene and aviation experts said the unusual configuration of the runway made landing there complicated.

It was reported by the Indian Government that eight passengers survived the plunge into the hilly terrain where the aircraft burst into flames. One survivor stated, "as soon as we landed, the tire burst." Another survivor told reporters, "within three seconds there was a fire blast. The inside was filled with smoke."

The runway at Mangalore-Bajpe Airport (IXE) is built on a plateau and is surrounded by cliffs immediately beyond paved surface of runway. Indian aviation safety experts have expressed concern about safety standards in India's rapidly expanding aviation sector. Last year, there were three near-miss collisions between planes at the airport in Mumbai, India. Air India has been in the news for more that one safety related incidents. This includes a mid air scuffle between crew members and flight attendants during a flight. This altercation left the aircraft unmanned for minutes. In September of 2009 an Air India flight to Toronto was delayed while the crew searched for rats on board the aircraft.

Air India Express is owned by the Indian government and has reportedly been experiencing financial difficulties lately due to a decrease in air travel. Kapil Kaul, the chief executive of India and the Middle East at the Center for Asia Pacific Aviation, a research group, said pilot error was a likely cause.

The aircraft was relatively new. It was a Boeing 737-800 with having made its first flight in December 2007, some 2 years and five months before the fatal crash.

A Judge has appointed lead counsel in Toyota product liability claims.

A federal judge has appointed four of the most experienced product liability lawyers in the United Satates to the positions of lead counsel for two committees that will deal with the more than 320 lawsuits filed against toyota.
The first committee will handle all of the wrongfull death claims. The second committee will be the counsel for people who have filed lawsuits claiming a decrease in value of their cars since the begining of the recall.
To date, the Japanese automaker has recalled about 8 million vehicles.
Toyota has previously blamed floormats and sticky accelerator pedals as the cause of the sudden unintended acceleration that has lead to dozens of deaths.
The National Highway Traffic Saftey Administration has found that 52 deaths have been linked to the acceleration problems. This prompted the agency to fine Toyota a record $16.4 million that Toyota has already paid.
Justice for the consumers however, will be tough. As one of the lawyers put it, "We are up against a well financed and tenacious defendant that dosn't believe it owes consumers anything or has cars with defects."
The next step for the lawyers is to consolidate the lawsuits in the hopes that the federal judge will certify them as a single class.

Bus Crash In Clarksville TN Injures Driver

A school bus loaded with thirty elementary students was involved in a crash in Clarksville, Tennessee.

The wreck occurred at the intersection of Madison Street and Richview Road. It was reported that a teenage driver turned in front of the school bus while trying to turn into a shopping center parking lot.

The students were cleared by medical personnel and transferred to another school bus.

The driver of the other vehicle, who attends Clarksville High School, was taken to a local hospital and treated for minor injuries.

Toyota Executive had urged company to come clean before recall.

Irv Miller, one of Toyota's top U.S. public relations officials, sent an internal e-mail that urged a "coming clean" of knowledge about the companies defective accelerator pedals.

"We are not protecting our customers by keeping this quiet," wrote Irv Miller, group vice president for environment and public affairs. "The time to hide on this one is over."

This e-mail was sent five days before the massive recall.

"I hate to break this to you but WE HAVE A tendency for MECHANICAL failure in accelerator pedals of a certain manufacturer on certain models," Miller's e-mail began with several words in capital letters.

The e-mail was addressed to Katsuhiko Koganei, executive coordinator for corporate communications for Toyota Motor Sales U.S.A. Inc.

In a memo earlier that day, Koganei wrote Mike Michels, vice president of external communications, "Now I talked with you on the phone, we should not mention about the mechanical failures of acc. pedal because we have not clarified the real cause of the sticking acc pedal formally, and the remedy for the matter has not been confirmed."

These e-mail exchanges, among others, give weight to the theory that Toyota executives knew about the problems and potential dangers that the defects posed to customers a long time before they issued the recall.

Carthage TN Accident Injures Four

Four people were injured in a three-vehicle automobile accident that shut down the Carthage, Tennessee bypass just west of the entrance to Cumberland Mines.

Tennessee Highway Patrol Trooper Jimmy Tisdale, who investigated the crash stated that four people were traveling in the three vehicles and all were injured. Among the injured was a Gordonsville High School student.

Logan Bellar of Valleyview Drive, Gordonsville, TN was airlifted from a location near the scene of the wreck by Air Evac helicopter based in Lafayette, TN transported Bellar from the scene to Vanderbilt University Medical Center in Nashville, TN.

A second person involved in the accident was Nancy Bybee of Hurricane Branch, Gainesboro, TN. Bybee was transported by ambulance to Riverview Regional Medical Center in South Carthage, TN.

The third vehicle involved in the accident was carrying Amanda McCormick of Kempville Highway, Pleasant Shade, TN. McCormick's was also traveling with her minor daughter. The two were transported by ambulance to University Medical Center in Lebanon, TN.

It was Trooper Tisdale's opinion that the crash occurred when Bellar's vehicle crossed into the west bound lane, striking the other two vehicles head on. An accident reconstructionist from the Tennessee Highway Parole was dispatched to investigate the cause further.
